If this is your first dental appointment with us, please download, print and complete the new patient form and bring it with you to your appointment. Also bring:
- your dental insurance information
- a list of your medications
- your current medical history information
If you have any questions about the forms, please arrive early and we’ll be happy to assist. Note that first appointments take approximately one hour for a thorough examination including x-rays.
